Sick with mild illness? Need a medical certificate to show your employer or school? Request your medical certificate online. The fee for this service is $25.

We can generate a medical certificate for up to 5 days.

For your safety anything longer than five days will require an assessment.

If you are experiencing a more severe illness, please book a consultation with a doctor.

  • Please note this service is available to registered patients only!!

  • Available to casual patients with a face to face nurse appointment!

  • Only Off Work/Off School Certificates can be supplied.

  • All other medical certificates including WINZ, ACC or Drivers Licence Medicals require a regular appointment.

Step One | Request

Request your certificate by filling out the pre-consultation questionnaire. This questionnaire will give us an outline your symptoms and help us to ensure that treatment over the phone is a clinically safe option for you.

Step Two | Review

A clinician will review your request and contact you to make an assessment and discuss treatment.

Step Three | Complete

Your medical certificate will be delivered to you by email.
